IMPORTANT INSTRUCTION
1. Understand
properly to recognition, admission schedule, fee, instruction provided
on the council portal before filling the admission form.
2. Information
regarding the Eligibility rules, courses , subjects, fee structure,
free ship and scholarship etc. is available on the council portal. It is
a applicant’s responsibility to cheque all the information before
taking the admission to any courses.
3. In
case of Free ship and Scholarship if the applicants are failed to
submit/produce any of the required document he will not be able to claim
for Freeship and scholarship. CCAM will not accept the forms of those
applicant.
4. Applicant’s applying for admission should be present in person at nearest Admission office or H.O. at the time of filling the admission form.
5. Applicants can also download the application form through the website.
6. Fill
it with black ball pen only. It is expected that application form
should be clearly and completely filled . Avoid overwriting the letters.
7. For
the purpose of postal communication applicants are requested to fill
his complete postal address in application form. Board will not be
responsible for postal communication in case of address in wrong.
8. Applicant’s are requested to select the subjects carefully as the selected subject will appear on Admit Card.
9. Fee should be paid by only Demand Draft. Fees once paid should not be refunded or not adjustable or changed under any rule.
10. Demand
Draft should be drawn in the favor of CHHATTISGARH COUNCIL OF
ALTERNATIVE MEDICINE and it should be payable at lormi (c.g.)
11. Attached all necessary original and attested true copies of the certificates with the application form.
12. Affix a recent passport size photograph in the appropriate box provided in the application form.
13. Candidates
are requested to verify all the necessary document as per the board’s
requirement and do not accept the form with incomplete document in any
case.
14. Admission
form filling is not a final status of admission confirm. All the
application form submitted at council will be checked and verified by
the council administration section.
15. After verification of document submitted by the applicants, council will issue Enrollment Number to
the eligible student only. The EN will be Communicated to applicants
through SMS and Email address provided by the applicants in the
application form.
16. The
Council/organization run non-formal self employment education program
only and not give any guarantee for further educational use or any job.
